EBV, is akin to the herpes virus and one of the most common human viruses worldwide. In fact, almost everyone becomes infected with EBV at some time during their lives. According to the Center for Disease Control, as many as 95% of Americans between 35 and 40 years of age have been infected. Many children become infected, but these infections either cause no symptoms or are indistinguishable from the other mild childhood illnesses.

In developed countries childhood infection is not as common. When the infection occurs during adolescence or young adulthood, it causes infectious mononucleosis in nearly half of reported cases. Symptoms of mononucleosis almost always include fever, sore throat, and swollen lymph glands, and occasionally a swollen spleen or liver may develop. In rare instances, heart problems or impairment of the central nervous system occurs, and in very rare cases, infectious mononucleosis can be fatal.

Although the symptoms of infectious mononucleosis typically subside in one to two months, EBV will remain dormant in the blood, throat and immune system for life. Periodically the virus can reactivate and can be found in the saliva of infected people. This reactivation usually occurs without symptoms. It is in this way that healthy people carry and spread the virus intermittently for life, rendering transmission of the virus is almost impossible to prevent.

Transmission of EBV occurs during exchange of saliva with an infected person. The time from infection to appearance of symptoms ranges from four to six weeks. Persons with infectious mononucleosis may be able to spread the infection to others for a period of weeks.

It is important to note that symptoms related to infectious mononucleosis caused by EBV infection rarely last for more than four months. At the six-month mark, it is frequently called chronic EBV infection and should be investigated further to determine if it meets the criteria for chronic fatigue syndrome, or CFS.

Diagnosis
In most cases of mononucleosis, a diagnosis can be made from the long-term presence of the three main symptoms, fever, pharyngitis, and lymphadenopathy. Beyond that, blood testing is used. Interpretation of laboratory results is somewhat complex and should be left to physicians who are familiar with EBV testing and who have access to the patient's background.
